Partners in Care (Diabetes Management)

What is Partners in Care?

Partners in Care is an educational program for people with diabetes. The program will focus of ways to control blood sugar and live healthy with diabetes.

Participants will meet in a group every week for 12 weeks. Each meeting is about 1 hour and will be held in your community.

Topics of the program include:

  • How food and exercise affect blood sugar,
  • How to avoid diabetes complications,
  • How to get support from friends and family for your diabetes, and
  • How to talk with your doctor.

What are the benefits of Partners in Care?

You will learn how to better manage your diabetes and stay healthy. There will be hands-on activities, educational materials, group support, and you will learn how to live healthy with diabetes.
